As a Senior Staff Nurse (Home Care), you will be responsible for the following duties:
- Providing home visits to patient for symptom control and assessment.
- Providing psychosocial support to patient and family and educating them on appropriate care of patient at home as needed.
- Develop, plan, implement and evaluate nursing care plan for patients
- Carry out appropriate nursing care according to patient’s needs
- Evaluate and monitor patient’s progress in relation to care plan
- Provide timely interventions and treatment plans according to client’s need
Requirements
- Candidate must possess at least a Diploma in Nursing
- Registered with Singapore Nursing Board
