Job Description & Requirements

Job Purpose

To manage the day-to-day operations and clinical delivery of the COMIT/MST programme within the designated zone, providing assertive community-based support to individuals with complex mental health needs and their caregivers and supervising the team in delivering high-quality, recovery-oriented services.

Job Responsibilities

A) Zone Operations & Case Management

  • Manage the daily operations of COMIT/MST within the designated zone, including caseload allocation, scheduling of home visits and team workflow
  • Maintain oversight of all active cases, ensuring care plans are current, appropriate, and documented to required standards
  • Providing direct case management and clinical intervention for high risk or complex cases within the zone
  • Ensure accurate and timely data collection, recording and outcome measurement across the team
  • Monitor and manage zone level budget, resources and programme expenditure

B) Assertive Outreach & Community Support

  • Ensure the team conducts regular home visits and community contacts in accordance with service model requirements
  • Coordinate outreach activities within the zone, including liaison with referral agencies, community partners and family members
  • Support clients and caregivers through psychosocial intervention, individual and group work, crisis support as well

C) Crisis Response

Lead the team's response to crisis situations within the zone, including risk assessment, safety planning and coordination with hospitals and emergency services

Ensure all team members are competent in and adhere to crisis response protocols

Escalate high-risk situations to Head, Comit/MST in a timely and appropriate manner

Qualifications & Experience

  • Degree in Social Work or any related discipline.
  • Minimum 6 years of experience in managerial experience in supervising centre or programme operations is preferred.
  • Background in programme management, learning and people development, social work or service, psychology, case management is advantageous.
  • Possess strong interpersonal and communication skills
  • Experience in mental health or social service organization will be an added advantage
  • Willing to work on some weekends, when required.
  • Experience in a non-profit, healthcare or social service organization will be an added advantage.
