Working Hours: 5 days, office hours

Occasional weekends: Required for community events

Contract: 1 year convertible to Perm

Conduct health and care assessments to understand residents’ health status, needs and lifestyle risk factors.
Engage residents in health goal-setting using the S.M.A.R.T. framework.
Develop personalised health coaching plans based on individual needs and wellness goals.
Guide residents in making sustainable lifestyle and behavioural changes.
Support residents with chronic disease self-management and health risk reduction.
Monitor, track and document residents’ progress and health outcomes.
Work closely with the Community Health Team to coordinate holistic care plans and service delivery.
Participate in community outreach programmes and health-related events, including occasional weekend events.

Min A level or Diploma Nutrition / Dietetics/ Biomedical Science / Biomedical Engineering/ Nursing / Health Sciences or related field
