Working Days: 5 days/week (Office hours)

May be required to work on weekends occasionally for community events.

Conduct comprehensive health assessments to understand residents' health status and risk factors
Engage residents in setting personalised health goals using the S.M.A.R.T. framework
Develop tailored health coaching plans to support residents in achieving their wellness goals
Guide residents in making sustainable lifestyle and behavioural changes
Support residents in managing chronic health conditions and reducing health risks
Track and document residents' progress and health outcomes
Work closely with the Community Health Team to support holistic care planning and service delivery
