Perform basic assessments to identify eligible patients for the study.
Perform consent-taking, baseline and outcome assessment.
Administration of study questionnaires.
Perform 6-monthly telephone calls for outcomes during the study period.
Coordinate the care for patients with other providers (health coach, dietitian etc.).
Arrange for follow-ups, laboratory tests and procedures as scheduled or ordered by the care team.
Perform accurate research documentation
Send reminders for appointments and track, recall and reschedule of defaulted appointments.
Manage subject reimbursement and ensure proper tracking of budget utilisation.
Facilitate the completion of Physician Management Checklist by Physicians.

Requirements:

Bachelor Degree in Nursing, or Bachelor Degree in Life Science, Medical Sciences and/or related medical fields.
Proficient in Microsoft Office applications.
Prior knowledge in NGMER and prior experience as CRC will be an added advantage.
