Senior Social Worker #HJO

Job Responsibilities

• Conduct psychosocial assessments to identify the needs of clients/patients and their families.
• Provide counselling, emotional support and appropriate social interventions.
• Develop and implement individualised care plans based on identified needs.
• Provide case management and follow-up to ensure continuity of care and support.
• Assist clients and families in accessing relevant financial assistance, social services and community resources.
• Coordinate referrals to appropriate healthcare, social service and community agencies.
• Work closely with multidisciplinary teams and external partners to support clients' overall well-being.
• Provide support and guidance to caregivers and family members.
• Maintain accurate case documentation, assessments and intervention records.
• Participate in case conferences, care planning and multidisciplinary discussions.
• Support programmes, outreach activities and other social service initiatives where required.

Requirements

• Degree or Postgraduate Diploma in Social Work from a recognised institution.
• Relevant experience in healthcare, community care or social services will be advantageous.
• Good knowledge of community resources and social assistance schemes.
• Strong assessment, counselling and case management skills.
• Good communication and interpersonal skills.
• Able to work effectively with clients, families, multidisciplinary teams and community partners.
• Empathetic, organised and able to manage cases independently.
