Facilitate Advance Care Planning discussions with patients and families
Coordinate ACP documentation and ensure accessibility for healthcare teams
Promote ACP awareness through education, training, and community engagement
Monitor ACP outcomes, maintain reports, and support quality improvement initiatives
Collaborate with multidisciplinary teams and community partners to enhance ACP services
Support ACP events, projects, research, and programme development

Bachelor's Degree in Nursing, Psychology, Social Work, Counselling, or related Allied Health discipline
