Salary: Up to $4,000 – $5,000 (Based on experience)

Facilitate Advance Care Planning (ACP) discussions with patients, families, and nominated healthcare decision-makers to understand and document patients’ care preferences.
Ensure timely accessibility and proper maintenance of records for healthcare professionals and care providers.
Provide follow-up support to patients and families for review and updates of decisions when required.
Advocate and promote awareness of among patients, caregivers, healthcare professionals, and community partners.
Assist in organising events, training sessions, meetings, and community outreach activities.
Collaborate with multidisciplinary teams including doctors, nurses, allied health professionals, and community partners.
Support quality improvement projects, research initiatives, and process enhancement related to services.
Contribute to educational materials, newsletters, and outreach initiatives to increase awareness in the community.

Bachelor’s Degree in Psychology, Nursing, Social Work, Counselling, Allied Health, or related disciplines.
