Occupational Therapist Grade II

Responsibilities

• Deliver Occupational Therapy services for inpatient, outpatient, and home care patients.

• Assess, develop, implement, review, and document individualized rehabilitation plans and discharge planning.

• Monitor patient progress, provide follow-up care, and ensure compliance with clinical guidelines.

• Collaborate with multidisciplinary teams, patients, families, and caregivers to support rehabilitation goals.

• Provide caregiver education and ensure safe, evidence-based therapy interventions.

• Maintain accurate clinical documentation, daily billing, and caseload management.

• Ensure safe use of therapy equipment and report incidents or equipment issues.

• Participate in case conferences, staff training, health education, and community outreach activities.

• Support departmental initiatives, continuous professional development, and quality improvement.

• Assist with patient engagement activities and contribute to service development.

Requirements

• Diploma, Bachelor's, or Master's in Occupational Therapy recognized by the Singapore Allied Health Professions Council (AHPC).

• Registered with AHPC with a valid Practising Certificate.

• BCLS and AED certification.

• 1–2 years of Occupational Therapy experience in a hospital, nursing home, or community/aged care setting preferred.

• Strong communication, interpersonal, critical thinking, and teamwork skills.

• Able to work independently, manage multiple priorities, and demonstrate leadership or mentoring abilities.

• Proficient in Microsoft Office and comfortable using healthcare information systems.

• Passion for geriatric rehabilitation and willing to travel to patients' homes when required.
