What You'll Be Doing

• Review and dispense medications accurately in accordance with professional standards

• Provide medication counselling and pharmaceutical care to patients

• Conduct inpatient medication reviews and support acute care services

• Collaborate with doctors, nurses, and allied health professionals to optimize patient outcomes

• Monitor medication safety, appropriateness, and therapeutic outcomes

• Maintain accurate clinical documentation and comply with regulatory standards

• Participate in quality improvement and medication safety initiatives
