Job Responsibilities

• Review and verify prescriptions to ensure appropriate medication selection, dosage, route of administration and duration of therapy.
• Dispense medications accurately and efficiently in accordance with established pharmacy policies and regulatory requirements.
• Provide medication counselling to patients and caregivers on the safe and effective use of prescribed medications.
• Identify, resolve and prevent medication-related issues, including potential drug interactions, contraindications and adverse drug reactions.
• Collaborate with doctors, nurses and other healthcare professionals to optimise medication therapy and ensure continuity of patient care.
• Respond to medication-related enquiries from patients and healthcare professionals by providing accurate drug information.
• Maintain accurate dispensing records and ensure compliance with hospital policies, professional standards and regulatory requirements.
• Support medication inventory management, including stock replenishment, expiry checks and safe storage of medications.
• Adhere to medication safety, infection prevention and workplace safety standards.
• Perform other pharmacy-related duties as assigned by the reporting supervisor.

Requirements

• Min Bachelor Degree in Pharmacy recognised by the Singapore Pharmacy Council (SPC).
• Registered Pharmacist with a valid Practising Certificate issued by the Singapore Pharmacy Council.
