Senior Social Worker

The Senior Social Worker will play a pivotal role in offering comprehensive psychosocial assessments and case management for clients facing complex social and healthcare challenges. This position emphasizes support for clients and caregivers to promote safe and dignified aging within the community.

Responsibilities

• Conduct thorough psychosocial assessments for clients and caregivers.

• Develop and implement individualized care plans tailored to clients' needs and aspirations.

• Provide emotional support, crisis intervention, counseling, and practical assistance to clients and families.

• Manage complex cases independently while monitoring progress and regularly reviewing intervention plans.

• Facilitate referrals and coordinate services with healthcare providers, community partners, hospitals, and government agencies.

• Advocate for client needs, ensuring access to community resources and financial assistance options.

• Support caregivers in managing caregiving stress and challenges through educational initiatives and family meetings.

• Collaborate closely with healthcare professionals, participating in multidisciplinary meetings and care planning discussions.

• Provide training, mentorship, and clinical supervision to junior social workers, interns, and support staff.

• Maintain accurate case documentation and ensure compliance with organizational policies and regulatory requirements.

• Engage in programme planning and quality improvement initiatives to enhance service delivery and client outcomes.

Requirements

• Relevant qualifications in Social Work or a related field.

• Minimum of five years of experience in social work or a related area.

• Proven ability to manage complex cases and deliver person-centred care.

• Strong communication and interpersonal skills for effective collaboration with multidisciplinary teams and clients.

• Familiarity with community resources, healthcare systems, and service delivery frameworks.
