Job Responsibilities:

Conduct home visits to provide nursing care for patients with chronic illnesses and palliative care needs.
Perform comprehensive nursing assessments and develop appropriate care plans.
Coordinate with doctors on patients' clinical conditions, medical assessments, and follow-up care.
Administer and monitor prescribed medications, ensuring compliance with treatment plans.
Monitor patients' progress and implement appropriate nursing interventions.
Educate patients, caregivers, and family members on home nursing care and disease management.
Maintain accurate clinical documentation and update patient records using electronic systems.
Collaborate with the multidisciplinary healthcare team to ensure continuity and quality of patient care.
Perform other nursing duties as assigned.

Requirements:

Registered Nurse with a valid Singapore Nursing Board (SNB) practising certificate.
Palliative care training and experience in home care or community nursing.
Competent in performing clinical nursing procedures independently.
