Explore and provide psycho-social/emotional care and support to patients and caregivers/family.
Lead in ACP discussions and completion. Complete Preferred Care Plan
Provide Bereavement Support.
Coordinate in-house Financial Assistance for needy patients.
Coordinate Medical Escort & Transport for patients.
Coordinate in referring patients for Respite Care, Nursing Home and other necessities.
Counselling/ Reconciliation
Crisis Intervention
Education, Teaching & Networking

Degree or postgraduate diploma in Social Work
Preferably 2 years relevant experience
Must have interest to work with elderly
Displays compassion, a positive and caring attitude towards patients, internal and external clients.
Excellent communication, interpersonal and problem solving skills
Meticulous, diligent and a good team player.
Adaptable and comfortable with changes
