Wellness Coach (Healthcare)

• Plan, promote, and implement wellness programs and activities at the community centres.
• Conduct screenings, physical assessments, and track patient progress with follow-ups.
• Monitor patient participation and compliance with individualized care plans.
• Maintain and disinfect program equipment regularly.
• Collaborate with clinical teams to support patient engagement and wellness outcomes.

Requirements:

• Diploma in Health Sciences (Health Management), Sports Science, or equivalent.
• At least 2 years of experience in healthcare, community care, or wellness program facilitation.
• Passionate about community service with a proactive and organized approach.
• Strong interpersonal and communication skills.
